Dean’s Causes of Death: In the time-loop episode, Dean was killed approximately 103 times, as part of the Trickster’s doing. In the deaths that viewers saw or heard about, Dean was shot at the Mystery Spot; was hit by a car; was crushed by a falling desk; choked on sausage; slipped in the shower; got food poisoning from a taco; electrocuted himself; was accidentally axed by Sam; was shot with an arrow; was mauled by a golden retriever; and was shot by a mugger.
Resurrected By: Gabriel the Trickster, after much pleading from Sam.

Dean’s Cause of Death: Torn apart by hellhounds when Lilith collected on the deal Dean made with a crossroads demon to save Sam’s life.
Resurrected By: Castiel in “Lazarus Rising” (Season 4, Episode 1) on God’s orders, because Heaven had work for Dean to do.
